NCFS Small Business Grant Application

This is a pre-qualification/application form for the NCFS Reimbursable Small Business Grant. You must complete the Business Intake Form here if you haven't done so already to become a NCFS Business Services Client. 

To complete your application, you'll need to provide several documents, including a Business W-9, Business Plan, Business Oregon Certification Form, and a photo ID.  Blank copies of these forms available within the application.

This grant offers reimbursement for eligible business expenses up to $5,000. To qualify, you must submit itemized receipts with this form. Please note that additional funding approval outside of NCFS is required, which may take up to 4 weeks before reimbursement funds are distributed.
